Volunteer Agreement on Behalf of Volunteer/Volunteer Group

Dress Code All volunteers must be dressed and groomed in a manner that is appropriate to their working conditions at all times.

Due to allergies and sensitivities to particular odours, perfumes and scents, volunteers are asked to minimize the use of perfumes, aftershaves, colognes and scented lotions. ‘Fragrance-free’, ‘scent-free’ or ‘unscented’ versions of personal care products are encouraged.
All clothing must be clean, neatly pressed and modestly fitted. See-through clothing is not permitted.
Bare mid-drifts (around the abdomen) may not be exposed and low cropped tops, miniskirts, tank-tops or spandex are not permitted.
Exercise clothing is also not permitted unless it is required for the work/activity performed.
Shoes must be clean, comfortable, safe and appropriate for the work being performed, i.e. closed toed shoes and steel-toed shoes where needed.
Undergarments may not be exposed.
Shorts, with the exception of short-shorts, may be worn during the summer months.
Jeans must always be in good repair and never torn or tattered.
Clothing that promotes alcohol, drugs and sex or promotes violence or other forms of oppression are not permitted. Profanity and/or put-downs may not be displayed on any clothing or accessories.
Should a volunteer report to work improperly dressed or groomed, the department supervisor may instruct the volunteer to return home to change. Volunteers will not be permitted to work when they are improperly dressed.

I hereby grant to Good Shepherd the right to reproduce, use, exhibit, display, broadcast and distribute still photographed or moving images, as well as interviews (audio and written transcripts) and published articles, for use in connection with the activities of Good Shepherd or for publicity, public education and fundraising purposes. This grant includes, without limitation and in perpetuity, the right to publish such images to Good Shepherd websites, in Good Shepherd publications, and in related educational/promotional/marketing/ public relations materials, such as media releases, posters and advertising. These images may appear in any of the wide variety of formats now available and that may be available in the future, including – but not limited to – print, broadcast, videotape, DVD, CD-ROM, digital and electronic/online media.

Confidentiality Agreement

Whereas the nature of services provided by Good Shepherd Hamilton and the conditions of governmental funding, privacy legislation and Good Shepherd policy requires that information be handled in a private and confidential manner;

 I understand that in the course of the performance of my duties as a student, employee, contractor, volunteer, program participant or associated member retained by Good Shepherd, I will be working with, exposed to and/or have access to Confidential Information and agree that I will not use the Confidential Information for any purpose other than in the normal course of my responsibilities without the express written permission of the Good Shepherd.

 I further understand and agree that information about our clients will not be released to outside individuals or agencies or to unauthorized persons within the agency without the written consent of Good Shepherd and/or the consent of the client except to follow legal or emergency guidelines. All memoranda, notes, reports or other documents compiled about our clients will remain part of the confidential records of Good Shepherd.
 In addition, personal or identifying information about Good Shepherd staff and volunteers (such as, but not limited to, names, addresses, phone numbers and salaries, health and other personal information) that is known by me through the course of my employment/association with Good Shepherd will not be released to persons not authorized by the nature of their duties to receive such information. Such information will not be released to outside persons or agencies without the consent of the staff person or volunteer; the only exception to this policy will be to meet legal requirements in an emergency.

 I further agree that breach of this agreement will subject me, where applicable, to disciplinary action up to and including the termination of my volunteer role or association with Good Shepherd as the case may be and that if there are any questions directed to me regarding the disclosure of information, I will refer the matter to my immediate supervisor.

 I further agree to hold in confidence any and all information and materials relating to or arising out of the Projects or work performed by me for Good Shepherd except for Confidential Information which was in the public domain or enters into the public domain through no improper act on my part. 

I further agree that no photographs of clients/guests/residents, staff, volunteers or placement students may be taken without permission of those being photographed. 

I understand that any breach by me of the above obligations, resulting in the wrongful disclosure or use of the Confidential Information in the course of my employment/association with Good Shepherd may give rise to irreparable injury to the Good Shepherd and may subject me to substantial money damages and legal proceedings.
