Seniors at Risk in Hamilton, funded by Ontario Health-West, is a collaboration between Good Shepherd Centres, the Alzheimer Society of Hamilton and Halton and St. Matthew’s House.
Seniors at Risk in Hamilton offers a centralized referral and intake process to facilitate timely interventions and supports for seniors impacted by issues such as dementia, abuse/exploitation, mental and physical health challenges, precarious housing, food insecurity and social isolation.
Seniors at Risk in Hamilton Supports:
- Older adults in Hamilton (ages 55 +).
- Older adults that are vulnerable and at risk of abuse, neglect and self-neglect, lonely and isolated without supports, physical health challenges.
- Older adults living with cognitive impairment/dementia (with or without diagnosis); caregivers.
- Older adults from diverse cultural and ethnic backgrounds who are underserved.
- Older adults that are facing evictions, home safety concerns related to hoarding issues.
Services Offered:
Case Management
- Timely case coordination and intervention.
- Connecting and collaborating with community agencies.
- Advocacy and education especially those experiencing abuse.
- Assist with housing & benefits applications. Income tax filing etc.
Dementia and Caregiver Support
- Support to persons living with dementia and their caregivers, so they can live safely in their community.
Homelessness Prevention and Intervention
- Provide assistance and support that address housing, hoarding issues and eviction prevention.
- Legal support pertaining to Landlord Tenant Board issues.
- Connecting vulnerable seniors to emergency food and prepared meals.
Volunteer Telephone Support
- Interactive programs for mental, social and emotional well-being.
- These programs are delivered entirely over the telephone by peer volunteers and there is no cost for participants.
- This free service is available for older adults who are 55+.
Virtual Senior Group
- Aims to connect seniors in our community virtually once a month via Zoom.
- Many older adults in our communities are socially isolated and unable to access in-person program due to physical, transportation difficulties and other barriers.
Seniors Wellness Group
Location: 400 King St. W Hamilton
Good Shepherd Community Room
Date: Every 2nd & 4th Thursday of the month, 3p.m.-4:00p.m.
The purpose of the group:
- Encourage older adults to participate in community engagement and build social network,
- Connect older adults with resources and supports in the community.
- Reduce social isolation
- Promote healthy living
- Provide educational workshops related to older adult safety, government benefits, healthy eating and information to help them make informed life decisions.

Referrals to Seniors at Risk in Hamilton are managed through a central access point and reviewed by representatives from Good Shepherd, the Alzheimer Society, and St. Matthew’s House. Each referral will be assigned to the appropriate collaborative partner.
