Homelessness Prevention

Reaching Home’s Homelessness Prevention program supports youth, women/nonbinary adults, and families at imminent risk of homelessness to maintain their current housing by providing landlord advocacy, tenant education and promoting housing stability. Participation in the program is voluntary.

Services & Supports

• Eviction Prevention & Shelter Diversion: Providing supports and interventions to stabilize and maintain tenancy
• Community Outreach & Case Management: Intensive short-term intervention, support, community referrals, education and advocacy
• Tenant Education: Increase knowledge of tenant rights and responsibilities
• Advocacy: Build and maintain positive relationships with landlords through mediation, advocacy and support
• Financial Assistance: Exploring avenues for financial assistance to support first and last months’ rent, rental arrears, utility arrears and other areas of instability

Program Eligibility

• Youth, Women/Non-binary Adults and Families are housed, and at risk of homelessness within 60 days
• At risk of eviction, housing loss, facing barriers to maintaining housing stability
• Participants can remain safely in current housing
• Self or community referrals

 

Housing First Programs

Reaching Home’s Intensive Case Management (ICM) and Rapid Rehousing (RRH) programs support homeless youth, women/non-binary adults, couples and families to secure permanent housing while facilitating supports that promote housing stability.

ICM & RRH Supports

• A person-centred approach to service delivery
• Clients are supported in their efforts to view and apply for units, including support with signing leases and moving
• Monthly in-home visits with clients to promote housing stability
• Support with referrals and connections to other community partners, dependent on the individual goals of the client
• Intergrated health team, such as nurse practitioner, registered practical nurse, mental health clinician, and harm reduction worker

Program Eligibility

• Referrals come directly from the City of Hamilton’s Coordinated Access Priority List
