HOMES (Housing with On-site, Mobile and Engagement Services) is a supportive housing program funded by the Ministry of Health and the Local Health Integration Network. HOMES provides housing and supports to people living with mental health issues who have experienced homelessness or are at risk of being homeless.

HOMES

HOMES (Housing with On-site, Mobile and Engagement Services) provides housing and supports to people living with mental health issues who have experienced homelessness or are at risk of being homeless.

Using on-site and mobile teams, support is delivered in a person-driven approach using:

  • Housing First best practice
  • Recovery
  • Harm Reduction approaches

Tenants in the HOMES program receive ongoing intensive supports designed to enhance their quality of life and assist in eliminating the stigma experienced by those living with a mental health issue. Tenants have access to a variety of supports including:

  • Rent-geared-to-income housing
  • Life skills development
  • Peer support
  • Health and wellness
  • Social recreation
  • Access to health care
  • Addictions supports
  • Community integration
  • Vocational supports

The program funded by the Ministry of Health and Long-Term Care and the HNHB Local Health Integration Network.

Community Homes for Opportunity (CHO)

In 2021 the Ministry of Health requested that Good Shepherd HOMES incorporate the ministry-funded Homes for Special Care program into our supportive housing program. The HOMES-CHO program is operated though transfer payment agreements throughout Hamilton and the Niagara Region. CHO homes are run by independent third-party vendors that provide on-site supports 24 hours a day, seven days a week in congregate care settings. Residents in these settings receive additional supports provided directly through the HOMES-CHO team. There are five CHO Homes operating in the Hamilton/Niagara  region:

  • Community Residential Care Home (Hamilton ON)
  • Green Mountain Home (Hamilton ON)
  • Rukavina Care Home (Hamilton ON)
  • Lincoln Lodge (Town of Lincoln ON)
  • Montebello Palace (St. Catharines ON)
